This article explains how Administrators can update existing Client Demographic forms. add new forms, and add new form elements.
Selecting the Desired Demographic Form to Edit
- On the right side of the top navigation, click the user icon, and then click Edit Client Template.
2. Under Which version would you like to edit? Select if you want to edit the Current Published Version or a Saved Draft.
Reordering the Form Elements
The form elements on each page of the client demographic form (client intake) can quickly be reordered using a drag-and-drop interface. To reorder the elements on the form:
- Hover over the blue arrow on the left of the desired element. You will see the cursor change from a hand with a pointing finger to a hand with a fist.
- Without releasing the cursor, drag the element up or down until it is in the desired order.
Note: To see what the client demographic form looks like with your changes, click Preview with Changes.
3. Once all of the form elements are in the desired order, click Publish.Adding a New Element to the Form
On the left side of the page under Drag and Drop Form Elements, you can see a variety of element types that you can choose to add to your form. To add a new element to the form:
- Click on the desired element. You will see the cursor change from a hand with a pointing finger to a hand with a fist.
- Without releasing the cursor, drag the element to the right side of the Client Demographic Form Builder under Drop Elements Here.
3. Without releasing the cursor, drag the element up or down until it is in the desired order.
Note: To see what the client demographic form looks like with your changes, click Preview with Changes.
4. Once the new element is in the desired order, click Publish.Editing a Form Element
- On the right side of the Client Demographic Form Builder, under Drop Elements Here, click on the element you want to edit.
- Each type of element has different characteristics you can edit. For example, the screenshot to the right shows how you can edit the Postal Code element, including:
- Label Text – This is the actual element name that will appear on the form once published.
- Placeholder Text – Enter any desired placeholder text to help staff understand what must be entered in a specific field.
- Is the question required – This cannot be edited on the Date of Birth element, but you can choose whether or not you make other elements required.
- Is this personal identifiable information (PII)?
- Share this information with partners?
- If you select “Yes,” you will be prompted to search for a Partner Organization to add. Select the desired organization.
- Element Size – This enables you to choose the specific size of the element on the Client Demographic Form.
- Start question on new line – This enables you to move the question to a new line in order to help make the Client Demographic Form more readable for staff.
- Once you have made the desired changes to the form element, click Save.
Note: To see what the client demographic form looks like with your changes, click Preview with Changes.
- Once all of the form elements are in the desired order, click Publish.
