SureImpact Glossary

Activity 

An activity represents all of the organized services (i.e. counseling, case management, job training, food assistance) an organization provides to their clients to accomplish participant outcomes. An activity can contain a list of all services an organization provides to their clients. The activity list is located in the Client tab.   

Client 

In SureImpact, a client is the individual receiving direct services from a provider.  

Client-Based Program 

A program that tracks measures at a client level (i.e. measures that will be tracked over time to show an individual’s progress). For example, an organization could hold an English as a Second Language Class. If the class enables staff to track each individual’s English Language Proficiency, it is a client-based program. However, if the goal is to only track the number of participants in the class, it would be a non-client-based program.    

Client Demographic Form 

The Client Demographic form is used during the initial meeting with the client during which the case manager gathers basic client information for the individuals receiving services, such as name, contact info, education level, race, veteran status, housing status, etc. Staff can edit a client’s demographic information at any time. The actual Client Demographic Form can be modified by an administrator by clicking on “Edit Client Template”. 

Client History 

The Client History tab displays all the service activities and outcomes for an individual client. Staff are able to quickly review and update client progress in this tab. 

Cut off Score 

Outcome measures are used to assess a client’s current status. The cut off score is the number an organization considers to be the target to reach for each outcome measure. 

Form Elements 

A form element is a component that can be added to a form, such as a text field, checkbox group, dropdown, etc. Some of the form elements on the Client Demographic Form include the First Name text field, Race checkbox group, and Gender dropdown. You can quickly add or recorder the form elements on each page of the client demographic form using a drag-and-drop interface.  

Full Client Data Report 

A report that includes all added or modified client demographic information for all clients in an organization’s SureImpact system. 

Measure 

An outcome measure enables a staff member to set a goal for a client and measure the client’s progress over time. For example, a case manager for a youth mental health program may use the “Reduction in Disciplinary Action” measure in order to track how a youth progresses in school. Over time, the case manager would be able to track if the youth had an increase or a decrease in disciplinary action at school. 

Measure Date 

Each time a staff member records or updates a measure, the date is recorded. Staff members are able to view when each measure was obtained and see the client’s progress over time.  

Non-Client-Based Program 

A program that tracks measures at an aggregated level and the measure is not tied to individual client records. For example, an organization could hold an English as a Second Language Class. If the goal is to only track the number of participants in the class, it would be a non-client-based program. However, if the class enables staff to track each individual’s English Language Proficiency, it would be a client-based program.   
  

Outcomes 

An outcome measure enables a staff member to set a goal for a client and measure the client’s progress over time. The SureImpact Dashboard displays all client and organization outcomes in real time. Each SureImpact user has their own dashboard based on their role. For example, a frontline staff has a dashboard that displays the outcomes for their specific clients. A manager can see all of the client outcomes for all of their assigned staff.   

Program 

SureImpact is built to support the social-sector logic model of organizations, programs, services, and clients. An organization’s programs typically contain unique set of services, clients, and outcomes. For example, an organization may have a workforce development program with the goal to increase client wages. Within that program, an organization could have services such as a resume-building workshop that supports the goal to increase client wages.    

Service  

As mentioned previously, an organization typically has programs with specific goals, and these programs contain services to achieve the desired outcomes (i.e. case management services). Once a service is created in SureImpact, it is listed in the Client tab under “Activities”. 

SureImpact ID 

The SureImpact ID is a unique number automatically assigned to a client.
