How to Copy an External Measure Form Link to Share with Staff and/or Clients?

This article shows Administrators and Managers how to copy an External measure Form Link so they can share it with staff and or clients.

SureImpact enables you to send a link to an external form for tracking client-based and non-client-based Measures. The data submitted in the client-based Measure Forms are attached to a client by matching the First Name, Last Name, and Date of Birth.

The data submitted in the non-client-based forms are uploaded to the applicable program based on which program you are in when you you copy the link.

To copy the external measure form link:

  1. In the top navigation, click Programs.
  2. Select the desired Program.
  3. On the Measures tab, click Copy Form Link next to the desired measure. You will see a confirmation that you have successfully copied the form link.
  4. Click the Copy Form Link You will see a confirmation message that you have successfully copied the form link. You are now able to send the link to staff members and clients.
