How to Copy the Create Client External Form Link to Share with Staff and/or Clients

SureImpact enables the creation of new clients using an external form. A Manager or Administrator must copy the external form link and give it to staff or clients to use.

SureImpact enables you to send a link to an external form for doing a new client intake.

  1. In the top navigation, click Client.
  2. Click the Client Actions drop-down menu, and then select External Form Submissions.

  3. Click the Copy Form Link You will see a confirmation that you have successfully copied the form link.

Note: In order to protect Client privacy, there are a couple of changes to how the external form works when compared to how a user who was logged into SureImpact would use the form.

  • Duplicate Clients: Submitting the external form will not validate whether a client with the same first name, last name, and date of birth already exists in the system. Instead, you will receive a "duplicate client" error if you try to approve an external submission and that client already exists in your organization.
  • Adding Related Clients: The "Add Relationships" module will be disabled when the form is accessed by an unauthenticated user. Not all SureImpact environments contain the Adding Related Clients tab. If you do not see this tab when you are logged into SureImpact and click Create New Client, disregard this note.
